Certain tests require a valid order from a licensed healthcare provider. By submitting an order, you confirm that you are authorized to place it and that the information provided is accurate and complete. Inaccurate patient, insurance, or diagnosis information may delay processing or result in cancellation of an order.

Turnaround times shown on this website are typical estimates measured from receipt of an acceptable specimen at our laboratory. Times may vary based on test complexity, specimen quality, add-on testing, reflex testing, courier schedules, and circumstances outside our control. Specimens that are improperly collected, labeled, stored, or transported may be rejected and require recollection.

Prices listed on this website are subject to change and may not reflect your final responsibility. Cash-pay pricing applies only when insurance is not billed. Where insurance is billed, coverage, deductibles, coinsurance, and medical-necessity determinations are set by your payer, and you remain responsible for amounts not covered. Payment is due as stated on your invoice or at the time of service for cash-pay testing.

Portal accounts are issued to named individuals and may not be shared. You are responsible for keeping credentials confidential, for all activity under your account, and for notifying us immediately of any suspected unauthorized access. We may suspend or terminate access for suspected misuse, security risk, or violation of these terms.
